Print the below form and fill it out in your own handwriting using a pen. (Do not type) You will need to prove you are over 18 years of age, so you need to include a copy of your ID and send both the filled out form and copy of your ID along with the payment.

You must send your payment in the form of Cash, Check, or Money Order, I prefer Checks or Money Orders but will accept Cash, but we DO NOT take responsibility for lost orders, mail cash at your own risk.

Cash and Money Order payments are processed immediately Check Orders within 10 days of receiving these forms.
